Neonatal coughing / choking with first feed, suspected tracheoesophageal fistula
Recommended: Abdominal X-ray + Inability to pass NG tube + Bronchoscopy is the preferred imaging study for neonatal coughing / choking with first feed, suspected tracheoesophageal fistula. 5 main types; Type C (proximal EA + distal TEF) most common (~85%).
Recommended study
Abdominal X-ray + Inability to pass NG tube + Bronchoscopy
5 main types; Type C (proximal EA + distal TEF) most common (~85%). Air in stomach suggests distal TEF.
If the default doesn't apply
H-type TEF (rare, late presenting)
XR
Esophagram with prone positioning
Oral contrast
Watch-outs
VACTERL association
Image kidneys, spine, heart, limbs — vertebral, anorectal, cardiac, renal, limb anomalies in ~25%.
Pearls
- NG tube coiled in upper esophagus on CXR — diagnostic.
- Gasless abdomen suggests pure EA without TEF (rare).
- Definitive surgery shortly after diagnosis.
