Incidental misty mesentery / mesenteric mass, suspected sclerosing mesenteritis
Recommended: CT Abdomen with IV contrast is the preferred imaging study for incidental misty mesentery / mesenteric mass, suspected sclerosing mesenteritis. Fat misty mesentery + soft tissue mass / nodules + 'fat ring' sign around mesenteric vessels.
Recommended study
CT Abdomen with IV contrast
Fat misty mesentery + soft tissue mass / nodules + 'fat ring' sign around mesenteric vessels.
If the default doesn't apply
Atypical features / malignancy concern
NM
FDG-PET / CT + biopsy
IV contrast
Watch-outs
Differential includes lymphoma / carcinomatosis / IgG4 disease
Imaging cannot reliably distinguish; biopsy / clinical follow-up determines.
Pearls
- Fat ring sign: preserved fat around mesenteric vessels in a soft tissue mass.
- Tumoral pseudocapsule: hyperdense rim around mesenteric mass.
- Often incidental and asymptomatic; treatment for symptomatic disease: steroids ± tamoxifen.
