Benign bone lesion in adolescents and young adults. < 2 cm nidus with reactive sclerosis. Common in long-bone diaphysis, posterior elements of spine.

Painful scoliosis in adolescents
Painful scoliosis = osteoid osteoma until proven otherwise; image the spine carefully.
Pearls
- Pain worse at night, relieved by NSAIDs (prostaglandin-driven).
- Radiofrequency ablation under CT guidance is curative.
- Osteoblastoma is similar histology but > 2 cm, may behave more aggressively.
