Medial ankle pain after ankle inversion — include proximal fibula X-rays to exclude high fibular fracture.

Pearls

  • Ottawa Ankle Rules: pain in malleolar zone + bony tenderness at malleolus posterior edge / tip OR inability to bear weight 4 steps → image.
  • Always include Mortise view to assess syndesmotic widening.
  • Salter-Harris fractures in skeletally immature patients — be alert for occult physeal injury.
