First-trimester bleeding + markedly elevated β-hCG, suspected gestational trophoblastic disease
Recommended: Transvaginal Pelvic Ultrasound is the preferred imaging study for first-trimester bleeding + markedly elevated β-hcg, suspected gestational trophoblastic disease. Complete mole (snowstorm), partial mole, invasive mole, choriocarcinoma spectrum.
Recommended study
Transvaginal Pelvic Ultrasound
Complete mole (snowstorm), partial mole, invasive mole, choriocarcinoma spectrum. Markedly elevated β-hCG.
If the default doesn't apply
Suspected metastatic GTD (invasive mole, choriocarcinoma)
CT
CT Chest + brain MRI if neuro symptoms
IV contrast
Watch-outs
Theca lutein cysts may rupture
Bilateral large ovarian cysts from hCG stimulation; supportive management.
Hyperthyroidism / preeclampsia
Markedly elevated β-hCG can cross-react with TSH receptor; preeclampsia in 2nd trimester is suspicious for mole.
Pearls
- Complete mole: snowstorm pattern, no fetus, β-hCG markedly elevated, 46XX (paternal).
- Partial mole: triploid (69XXY), fetus with anomalies, lower β-hCG.
- Post-evacuation β-hCG surveillance for chemotherapy decision.
