Watch-outs

Theca lutein cysts may rupture

Bilateral large ovarian cysts from hCG stimulation; supportive management.

Hyperthyroidism / preeclampsia

Markedly elevated β-hCG can cross-react with TSH receptor; preeclampsia in 2nd trimester is suspicious for mole.

Pearls

  • Complete mole: snowstorm pattern, no fetus, β-hCG markedly elevated, 46XX (paternal).
  • Partial mole: triploid (69XXY), fetus with anomalies, lower β-hCG.
  • Post-evacuation β-hCG surveillance for chemotherapy decision.
