Incidental mediastinal cyst, suspected bronchogenic cyst

Recommended: MRI Mediastinum (T2 hyperintense fluid signal) is the preferred imaging study for incidental mediastinal cyst, suspected bronchogenic cyst. Most common mediastinal congenital cyst.

Recommended study

MRI Mediastinum (T2 hyperintense fluid signal)

MRI IV contrast Preferred Chest Outpatient Reviewed

Most common mediastinal congenital cyst. Carinal / subcarinal location classic. Variable T1 signal depending on proteinaceous content.

Watch-outs

Hyperdense on CT can mimic solid mass

Proteinaceous content can mimic solid lesion on CT — MRI clarifies cystic nature.

Pearls

  • T2 markedly hyperintense, T1 variable (water = dark; proteinaceous = bright).
  • No enhancement; thin imperceptible wall.
  • Symptomatic, infected, or growing cysts: surgical resection.
